Required Documents
1. Child’s current immunization records
2. Note from doctor stating child has been seen in the last year and is healthy to attend childcare
3. Signed Discipline and Guidance Policy
4. Signed Parent’s Rights form
5. Signed Medical Release form
Documents as Needed
1. Allergy Action Plan – If your child has an anaphylactic reaction, please have your doctor fill this form out.
2. Sensitivity Document – If your child has a sensitivity to something, please fill out this form.
3. Infant Safe Sleep – Required for children who will be under one year old when they start.

1. Infant Care Sheet – for children under 2 years old
2. Child Care Sheet – for children over 2 years old but less than PreK 4
3. PreK 4 Care Sheet – for children 4 years old and over by September 1st
